Scholarship Application

By submitting this form, you are acknowledging and agreeing to the following conditions:

• You have read and fully understand the guidelines that govern the application and selection process, and you have provided answers to ALL questions that apply to you.
• You certify that all information contained in this form is true and correct. You understand that any false statements given on this application, by email or telephone, will disqualify your application and will affect your ability to access future financial support.
• You hereby give consent that ILC and AFOA Canada are authorized to release your contact information to Scholarship donors (including name, telephone number, email and mailing address) so that they may contact you personally.
• You understand that past recipients are not eligible to apply for the same scholarship.
• You hereby give consent for ILC and AFOA Canada to use/publish your name, photo, and relevant information on AFOA Canada and ILC’s website and other communiques for promotion, marketing, advertising of ILC and AFOA Canada.

Late applications will not be considered for this round but may be moved over to the next round, as applications are accepted on a rollover basis. Please keep a copy of your application package for you records.

By applying for an ILC scholarship, you are submitting your name for consideration for funding but it does not constitute application nor acceptance into an AFOA Canada program. In order to join a program or course, you must register through AFOA Canada’s application process. It is encouraged that you apply to the program of your choice before submitting a scholarship application.
